Sante SPS, the Health Programs Follow-up System, helped teams track patients enrolled in a program and step in early when something wasn’t going well.

A map of where cases concentrate.
Monthly trend of visits.

What I did

I designed and built a follow-up system so no patient in a program was lost from sight.
I gave teams statistics on visits, patients and repeat cases, and a map to see where cases concentrate.
